In 2018, an international group updated the language to line up with how neurocognitive disorders are described elsewhere in medicine (Frontiers in Neurology, 2025). The current framework describes a spectrum by timing:
- Postoperative delirium — an acute, fluctuating disturbance in attention and awareness, typically within the first week after surgery.
- Delayed neurocognitive recovery — cognitive changes identified within about 30 days of surgery.
- Postoperative neurocognitive disorder — changes recognized between roughly 30 days and 12 months afterward (Frontiers in Neurology, 2025).
The point of the newer terms is not to alarm but to be precise: a groggy, disoriented first night is different from subtle memory changes noticed a month later, and they call for different responses.
How Common Is It
The numbers vary widely depending on the surgery, the patient, and how carefully cognition is measured. A systematic review of older adults undergoing non-brain surgery found the prevalence of postoperative cognitive dysfunction ranged from 19% to 64%, with roughly 22% to 41% affected in the first week (Nursing Open, 2024). Postoperative delirium specifically affects an estimated 20% to 50% of older adults after major surgery, compared with under 15% in younger patients (International Journal of Molecular Sciences, 2025).
Interestingly, research suggests the type of surgery matters more than the type of anesthesia. Delirium risk tends to climb from abdominal procedures to cardiothoracic surgery to hip and femur repair, and no single anesthetic technique has proven clearly best for protecting the brain (International Journal of Molecular Sciences, 2025).
Who Is Most at Risk
Certain factors consistently raise the odds of cognitive changes after surgery (Frontiers in Aging Neuroscience, 2022; Clinical Interventions in Aging, 2025):
- Being over 65, and especially over 75.
- Pre-existing cognitive impairment or an earlier decline that had not been formally noted.
- Frailty, poor physical function, or depression before surgery.
- Longer or more extensive operations.
- Heavy alcohol use and lower formal education.

Here is the reassuring part. Postoperative delirium usually develops within hours to a few days and is often reversible as the body stabilizes (Clinical Interventions in Aging, 2025). For most people, the longer forms of cognitive change also ease over weeks to months. That same review notes, though, that recovery is not identical for everyone — a minority experience changes that linger, which is exactly why paying attention over time matters more than reacting to a single foggy afternoon.

- Line up support at home for the first weeks: familiar routines, good sleep, hydration, glasses and hearing aids, and gentle activity all support recovery.
- Track and report — and don't wait on acute changes. Sudden or fluctuating confusion, agitation, or disorientation in the first hours to days after surgery should be reported to the care team right away — while still in the hospital, or by contacting your clinician promptly if you are already home — because postoperative delirium can reflect a treatable complication (Clinical Interventions in Aging, 2025).
