That does not mean the effects are permanent for everyone; many people improve over months of abstinence, and study designs vary. But it does suggest that months or years of daily use is a different situation from an occasional dose.
Do Benzodiazepines Cause Dementia?
This is the question that worries most people, and the honest answer is that the science is unresolved.
The concern grew after a 2014 case-control study in The BMJ linked benzodiazepine use to Alzheimer's disease. Among older adults in Quebec, people who had used benzodiazepines had a roughly 50 percent higher odds of a later Alzheimer's diagnosis, with the association strongest for longer durations and longer-acting drugs (Billioti de Gage et al., 2014). Even those authors cautioned that benzodiazepine use might be an early marker of dementia rather than a cause.
That caution proved important. In 2016, a prospective study of 3,434 older adults in Seattle followed cognitive trajectories over years and found no dose-response pattern: the highest-exposure group showed no increased risk. The authors concluded their results "do not support a causal association between benzodiazepine use and dementia" (Gray et al., 2016).
Why the disagreement? A big reason is confounding by indication. Anxiety, agitation, and insomnia — the very symptoms benzodiazepines treat — can appear in the early, pre-diagnosis years of dementia. So people heading toward dementia may be more likely to be prescribed these drugs, making the medication look like a cause when it may be a consequence. For now, no consensus exists that benzodiazepines cause dementia, and that uncertainty is itself the takeaway.

Older adults face the clearest cautions.
